De-icing, car running, heater to max hot, vent controls to windscreen only, fan 3 or 4 (max fan isn't always the best, a slower airflow will pick up more heat as it has time to warm).
Outside, scrape off all the ice.
Then sit and shiver while the car clears the screen.
